A Case of Spontaneous Pneumomediastinum with Multiple Allergy Occurring as a Complication of Influenza Virus Infection

Abstract
Acute respiratory infection due to influenza virus can lead to lower respiratory infection, such as pneumonia, bronchitis, exacerbation of bronchial asthma or air leakage from the respiratory system. Pneumomediastinum is an uncommon condition which is usually related to trauma, an underlying lung disease and asthma. Spontaneous pneumomediastinum has been described in a few cases of influenza pneumonia. We report a ten-year-old boy with multiple respiratory allergy who was infected with influenza virus and developed spontaneous pneumomediastinum, pneumopericardium, pneumothorax and subcutaneous emphysema. He was admitted to the intensive care unit and treated with nasal O2 supplement, oseltamivir, empirical antibiotics and supportive care. Allergy skin prick test presented positive to multiple allergen including house dust mite (Dermatophagoides farina, Dermatophagoides pteronyssinus). His air leakage improved on the 5th day of admission. Herewith, we report a case of spontaneous pneumomediastinum occurring as a rare complication of influenza virus infection.
